影片讲述了民国时期，在上海求学的冯晨晨在参加学生运动中脑部受伤，在那之后每晚做梦都梦见同一个情景，深夜，水乡的河边，一个穿着红衣女人被一个无脸男人推进河里。男友宋子豪建议她回乡静养一段时间，于是她回到水乡甪直，没想到刚回老家便遭到纨绔子弟逼婚，甚至被恶少爷拖到河边的芦苇荡里强暴而跳河自杀。冯晨晨红颜薄命香消玉殒，但她“死后”却总以一身血红旗袍闪现在古镇水乡的幽暗街头、岸边水中或者树林深处，那个温柔可人的美丽少女就变成了一个冤魂不散的“女鬼”。血红旗袍的她诡异地出现，让整个水乡惊惧心寒这场“头七还魂”，村民们的惊吓和恐惧更非常人可比。在片中灵堂前的剧照中，旗袍血红，眼神凄冷，女鬼长发凌乱飘散，而其内心已经何等凄凉。
